Only eight swimmers qualified for the women's 400m individual medley at the 2020 Tokyo Olympics. In three heats at the Tokyo Aquatics Center, 12 swimmers competed in the butterfly, backstroke, breaststroke and freestyle events. Each swimmer had to complete 100 metres of each type of swimming in a specific order.

The following eight fastest swimmers in the pool moved on to the finals where they competed for the medals.

4. Mireia Belmonte 🇪🇸 4:35.88

6. Viktoria Mihalyvari-Farkas 🇭🇺 4:35.99
7. Katinka Hosszu 🇭🇺 4:35.01
8. Ilaria Cusinato 🇮🇹 4:37.37
