Sociologist Diane Vaughn's analysis of the Space Shuttle Challenger disaster found that NASA had normalized "deviant" behavior to the point that it no longer felt wrong to the people in the organization.

This lead to people ignoring vital danger signs and behaving collectively in increasingly risky ways.

While earlier analyses of the disaster point to individual failure points what Vaughn shows is that there were multiple points of failure and that trouble had been brewing for many years.

I made this video because I see this kind of normalization of deviance in many organizations (skimping on documentation, missing meetings, failing to retrospect and more).
