1.BIR Form 1901 (2 sets) duly accomplished. This includes details of accredited printer for the Authority to Print receipts with clear final sample of receipt/invoice , or in lieu of this, you can avail BIR Printed Receipt which is only valid for one (1) year.

2. DTI (if applicable/ if with trade name)

3. Photocopy of any Government issued ID with 3 specimen signature

4. selfie photo holding your government ID.

5. P30.00 worth loose Documentary Stamp which you can buy at our office
