You need
1. Yoshi
2. Make Yoshi eat a Spiny from the Lakitu
3. Spit out the Spiny between the non-breaking blocks
4. Now just stay put and the Lakity keep throwing Spinys
5. Keep an eye out for the Spinys falling!
99 is the most lives you can have!
