The purpose of this video is to highlight Trillium Community Health Plan’s partnership with Lane
County’s Shelter Care agency and their FUSE program to help address the problem of housing
insecurity among the most vulnerable population. Trillium CHP works with Shelter Care on
behalf of their members with behavioral and physical health issues to provide them the
necessary medical care they need, and help them find and secure long-term affordable and safe
shelter.
