SETTING Aged Care Facility
NURSE You are speaking to the son/daughter of an 82-year-old patient. His/her mother is in a wheelchair and has recently been transferred from an acute care facility to this setting with 24- hour care. You have just finished clinical observations and have discovered a pressure sore(bedsore) on the patient’s thigh. His/ her mother is not present for the discussion.
TASK
• Find out if son/daughter has any specific concerns about his/her mother.
• Give information about patient’s general health(well/happy, condition as expected. Advise of presence of
pressure/bedsores (eg: on thigh etc.) find out what son/daughter know about pressure sores.
• Explain pressure sores (damage to skin/underlying tissue, result of constant friction/ pressure on skin, due to: lack of
blood flow, muscular problems etc.)
•Outline possible causes of patient’s pressure sores (eg: pressure from wheelchair , fiction from clothing / bedding,
immobility, etc). describe immediate treatment (eg: cleaning of wound applying dressing, etc).
•Reassure son/daughter about your role in pressure sore prevention (eg: changing patient’s position every few hours:
relieving pressure, checking skin daily etc).
