Does biopsy or surgery cause the spread of cancer?
Dr. Sandeep B. Pillai, Surgical oncologist KIMS Cancer Centre, Trivandrum.

A cancer as we all know requires most of the time treatment in the form of surgery, chemotherapy or radiotherapy.
So to confirm the diagnosis even before you start the treatment is one of the most priorities in cancer treatment.
How to get confirmed diagnose of cancer?, there are various methods to do it if it is a easily accessible site, you can put in a needle and get some cells out of the cancer and examine them and make an accurate diagnose of cancer in about 80-90 percent of the cases.
But in many cancers getting a needle test is not enough to start the treatment, you need something more than few cells to confirm and even some cases to grade the treatment sometimes you may require a treatment other than surgery like chemotherapy or radiation therapy before the surgery.
So in all such cases it is prudent to get tissue for diagnosis, that is the role of a biopsy concept.
Previous years people use to remove a lump or a mass and subject it to pathological examination, which is of course the most short method of getting a diagnosis but unfortunately it just got a lot of consequences to the patient. For example a lump in the breast somebody removes the whole lump or the whole breast in some cases and wait for the report.
What happens in such cases is if there is chance to remove the lump without removing the breast they are losing the chance by doing a initial execution biopsy or removal of the old lump.
So it is a better way to approach the lump with a bigger needle to get more tissue than a needle test but it will be good enough to give you a confirmed diagnosis.
Such biopsy equipment is called a core cut or true cut biopsy and all the years it has been proven that a true cut biopsy will not result in any more spreading of the cancer than a needle test.
So it is a fairly safe procedure but there are certain areas where a biopsy can be considered a little more risker as far as the manner of cancer is concerned. One such example is welling in the form of a cyst within the abdomen if you do a puncture of the cyst there is a chance that cancer can spread through the beyond the needle tracks. So it is always better to consult a doctor and plan a biopsy accordingly. So in any case removal of a tumor for getting a biopsy is totally unacceptable in most of the time.
